The uncinectomy represents the first stage of endoscopic sinus surgery (ESS). Only uncinectomy allows the precise identification of the natural maxillary ostium and the exposure of the infundibulum ethmoidalis as natural drainage pathway of the anterior ethmoid and the frontal sinus. The identification of the maxillary sinus is performed using a curved curette or a curved aspirator. The meatotomy is widened in a dorsoventral direction, with retrograde bite forceps (antrumpunch backward), at the expense of the anterior fontanel area. The opening of the bulla must be performed, using the J-curette, in its inferomedial corner, a safety point distant from the papyrus lamina, placed laterally.
